The Two-Year Window India Cannot Afford to Waste

Trump's phased tariff on generic drugs gives India's pharma giants until August 2028 — then a wall. Behind the grace period lies a $9.7-billion export machine with a vulnerability hiding in plain sight.

Picture a pharmacist in Cincinnati reaching for a bottle of metformin — the diabetes pill taken by tens of millions of Americans. There's a good chance that pill was made in Hyderabad, Ahmedabad, or Baddi. Now imagine the US government slapping a 100% import duty on that bottle. The price doesn't just nudge up — it potentially doubles. And for a medication that already costs cents per tablet, the entire economics of the trade could unravel.

That is exactly what Donald Trump put on the table on July 21, 2026. The US President announced a 100% tariff on imported generic pharmaceutical drugs, set to go into effect in August 2028. For India, which has quietly become the indispensable backbone of American pharmaceutical access, the announcement is the loudest alarm bell the industry has heard in decades.

What Trump Actually Said — and What the Clock Looks Like

Trump's announcement is unambiguous: "Effective August 1st, 2026, all Generic Drugs being brought into the United States will continue to have a TARIFF of ZERO PERCENT for a two year period of time, after which the TARIFF will be raised to 100% for a one year period of time, and 200% thereafter."

The structure is deliberate. Generic drug manufacturers will have two years to move production to the US or face a 100% import duty from August 2028. The move is aimed at "reshoring" generic pharmaceutical production, and companies that don't build plants onshore will face levies that would then double, to 200%, in August 2029. The White House said the policy will be implemented under Section 232 authority — the same national-security trade powers used for steel and aluminium tariffs.

This is not the first pharmaceutical tariff shot Trump has fired. On April 2, 2026, Trump announced a Proclamation imposing 100% tariffs on patented pharmaceutical products and key inputs — one of the most aggressive trade actions affecting the life sciences sector. The Proclamation included broad exemptions, including for generic pharmaceuticals — and at the time, India's industry breathed a sigh of relief, since the overwhelming majority of India's US exports are generics. Tuesday's announcement closes that escape hatch, on a timer.

Two years of status quo. Then a wall.

Why India Is the One Most Exposed

India did not stumble into the generic drug business. It was built over decades — through process chemistry expertise, a deep pool of trained scientists, and a willingness to manufacture molecules at costs no rich-world competitor could match.

In FY25, India's generics dominated the US market, supplying 42% of all prescription drugs, up from around 21% in 2013. That doubling of market share in a decade is a story of relentless execution. India is often described as the "pharmacy of the world," supplying around one-fifth of global demand for generic medicines and exporting pharmaceuticals to over 200 countries, with approximately 70% of exports destined for highly regulated markets such as North America and Europe.

In dollar terms, the exposure is enormous. In FY2024-25, India exported around $9.7 billion worth of pharmaceutical products to the US, largely driven by generic drugs. During FY25, total exports of drugs and pharmaceuticals stood at $30.4 billion — making the US alone roughly 32% of India's entire pharma export machine. Convert that to rupees at current rates and you're looking at over ₹81,000 crore in annual exports from a single country, for a single category.

More than 90% of medicines sold in the US are generics, according to the US Food and Drug Administration. Americans depend on that volume to keep prescription costs manageable. India sits at the centre of that system.

The Brutal Economics of a Generic Pill

To understand why a 100% tariff is so potentially lethal for this trade, you need to understand how thinly generic drug manufacturing is actually priced.

Generic drugs are copies of brand-name medicines whose patents have expired. Because multiple manufacturers compete on the same molecule, margins get squeezed hard and fast. Generic drugs fill the overwhelming majority of US prescriptions by volume, yet represent only a fraction of total drug spending. All that volume, a thin slice of the money.

The fundamental contradiction is this: the same market forces that make generics cheap are destroying the conditions required to keep them available. An unrelenting race to the lowest unit price has compressed manufacturer margins to near zero — or below — for hundreds of products, and concentrated global manufacturing in a handful of overseas facilities.

Here is the crux of the tariff problem. A generic product already operating on a 5–8% operating margin has no slack to absorb a 100% tariff on the finished drug. The Indian exporter cannot simply eat it. Passing the cost to the American patient is politically toxic and practically difficult, given how PBMs — pharmacy benefit managers, the powerful intermediaries who negotiate drug prices between manufacturers, insurers, and pharmacies — operate. And exiting the market is the third option nobody wants to say out loud. None of these is a good outcome.

The Hidden Layer: India's China Problem

Here is where this story gets genuinely layered, and where Indian pharma's vulnerability is deeper than most headlines suggest.

India is celebrated as the "pharmacy of the world." But behind that label is an uncomfortable supply chain truth. The Department of Pharmaceuticals' annual reports have acknowledged India's heavy dependence on China for critical bulk drugs, with around 70% of such imports sourced from there. India's pharmaceutical manufacturing is built on a critical dependency on China for approximately 70–80% of its Active Pharmaceutical Ingredients (APIs) and Key Starting Materials (KSMs).

APIs, or Active Pharmaceutical Ingredients, are the chemically active molecules that actually do the therapeutic work in a drug. They are the most complex and capital-intensive part of the supply chain — and India largely doesn't make them domestically. In 2023-24, China accounted for 43.45% of India's pharmaceutical imports by value, amounting to $3.6 billion. This reliance is not limited to APIs but extends to key starting materials, chemicals, and solvents. For antibiotics, the dependence is near-total: by 2024, roughly 87% of India's imported antibiotic ingredients by value came from China — a figure that has risen sharply over two decades.

What this means in practice: India is a brilliant assembler and formulator of generic medicines, but the raw chemical inputs often originate in Chinese factories. India depends on China for approximately 70% of its bulk drug and intermediate imports. This indirect reliance means many pharmaceutical products originating from India still contain Chinese raw materials, making true diversification an ongoing challenge.

India is, in other words, caught in a double squeeze — dependent on China upstream for raw materials, and on the US downstream for finished products. This also complicates the Trump logic. His stated aim is to reshore pharmaceutical manufacturing to America. But "reshoring" generics without also reshoring API production is half a solution at best — because those APIs still originate in China. The supply chain problem is genuinely global and deeply entangled.

Can Indian Companies Just Build in America?

The honest answer: the big ones already have a foothold, but nowhere near enough capacity for what Trump is demanding.

India is home to the largest number of USFDA-compliant pharmaceutical manufacturing plants outside the United States — which tells you the capability exists. Sun Pharma, Dr. Reddy's, and Lupin all have US manufacturing or packaging facilities, built over the past two decades to stay close to the market. Leading Indian companies such as Dr. Reddy's Laboratories, Lupin, Sun Pharma, Glenmark Pharmaceuticals, and Zydus Lifesciences have significant exposure to the US market.

But running a distribution or packaging facility in New Jersey is very different from manufacturing generics at scale in the US. The cost economics are fundamentally different. US manufacturing requires American wages, American real estate, American environmental compliance — all multiples of what the same plant costs to run in Hyderabad. That cost gap is the entire reason Indian generics are cheap. Eliminating it doesn't reshore the industry; it destroys the value proposition of the product.

For the Divi's Laboratories and Sun Pharmas of the world, navigating this is painful but survivable — they have balance sheets, US regulatory relationships, and the ability to redirect capital. The real damage will fall on India's second and third tier of generic exporters: companies with revenues of ₹500–3,000 crore, deeply reliant on the US market, with no meaningful domestic US manufacturing footprint and no realistic capacity to build one in 24 months. These are the companies that generate the bulk of pharmaceutical employment and export-led forex earnings for India. If a 100% tariff makes their US business uneconomic in 2028, they don't pivot to the US — they scramble for Europe, Africa, or Latin America. That is not necessarily fatal, but it is a painful structural dislocation.

The Counter-View: Why This Might Not Play Out as Feared

A few reasons for measured scepticism before declaring catastrophe.

First, Trump's track record on pharma tariffs is messy. The world's biggest drugmakers signed deals with the US government that exempted billions of dollars of drugs from tariffs. Trump signed an executive order in April imposing 100% tariffs on branded pharmaceuticals unless manufacturers agreed to government drug pricing deals or committed to making their products domestically. Deal-making has blunted implementation before. The two-year grace period also, cynics would note, conveniently pushes the hard decision past the US midterm election cycle.

Second, the US cannot simply replace Indian generics. More than 90% of medicines sold in the US are generics, according to the FDA. Any supply disruption from tariffs lands directly on American patients — particularly the poorest ones who rely on Medicaid and Medicare. That is a political cost that any administration eventually feels.

Third, the legal and regulatory path to building new US generic manufacturing isn't quick. USFDA plant approvals, environmental clearances, workforce ramp-up — even companies that break ground today would struggle to have meaningful capacity by August 2028.

Fourth, India's PLI programme is real, even if it isn't yet sufficient. The government's Production Linked Incentive (PLI) scheme — which provides financial incentives for domestic manufacturing of critical pharmaceutical ingredients — has already exceeded its initial investment targets. As of September 2025, ₹4,763 crore has been invested under the scheme against a committed target of ₹4,330 crore, production capacities have been created for 26 KSMs/DIs/APIs that were earlier primarily imported, and cumulative sales of ₹2,315 crore have been reported, including ₹508 crore in exports — avoiding imports worth ₹1,807 crore. That is progress. But given the scale of what the US tariff threat represents, it needs to move faster and go deeper into the supply chain.

What Two Years Should Actually Look Like

The grace period is not a comfort zone. It is a runway.

For Indian pharma companies, the urgent priorities are clear: accelerate US facility investment for complex, high-margin generics where the economics can better absorb higher costs; file more ANDAs (Abbreviated New Drug Applications — the regulatory pathway to sell a generic in the US) for products with fewer competitors, where pricing power is greater; and diversify aggressively into Europe, Japan, and emerging markets to reduce the single-market dependency.

For the Indian government, the mandate is sharper. The PLI Scheme for Bulk Drugs aims to create domestic capacity for critical APIs, KSMs, and drug intermediates, thereby reducing dependence on single-source imports for essential medicines — but the scheme's tenure runs only to FY2029-30, and actual production capacity so far covers 26 molecules. The government has explicitly barred earlier PLI awardees who withdrew or had approvals cancelled for non-performance from re-applying for the same products — a sign of frustration with project delays and underutilisation in earlier tranches. The scheme needs more rigour, more capital, and a longer horizon.

Reducing API dependence on China is no longer just a strategic talking point. It is a prerequisite for sustaining the US export business. An Indian pharma company that reshores its US manufacturing while still sourcing APIs from Shandong has not solved its supply chain problem — it has just moved the vulnerability one step upstream.


The deepest irony of Trump's tariff is this: the medicines it would make more expensive are the ones that make American healthcare affordable. Generics — largely Indian-made — are what keep the US healthcare system from collapsing under its own drug bill. A 100% tariff on them is not a policy to protect American patients. It is a policy to make American patients pay more so that American factories can eventually replace Indian ones.

Whether those factories actually get built in two years is another question entirely. What is certain is that India's pharma industry, which has spent thirty years quietly becoming indispensable to American healthcare, now has twenty-four months to decide what it wants to be next.
